医学学术论文范文：【摘 要】 目的 探讨重症病毒性脑炎患儿的护理措施。方法 选取驻马店市第一人民医院2008 年7 月至2011 年7 月在内儿科住院的38 例重症病毒性脑炎患儿，按护理程序的步骤，对其实施整体化护理。结果 38 例患儿中，治愈34 例，好转2 例，放弃1 例，死亡1 例，治愈率。结论 重症病毒性脑炎不仅需要医师及时有效的救治，还需要护理人员细心的观察和实施早期的护理干预措施。 【关键词】 病毒性脑炎；重症；护理干预 病毒性脑炎系各种病毒引起的一组以精神和意识障碍为突出表现的中枢神经系统感染性疾病。病程大多2～3 周，多数完全康复，少数重症患儿易发生急性期死亡或遗留后遗症，致残率和病死率高[1]。重症患儿的临床表现有持续高热、头痛、恶心、呕吐反复惊厥发作、抽搐、不同程度意识障碍、精神情绪异常、病理征阳性、颅内高压甚至脑疝形成，导致呼吸衰竭而危及患儿生命。根据重症患儿病情重、起病急、变化快、进展快、并发症发生率高这些特点，如何提高患儿的护理质量，预防并发症成为我们亟待解决的问题，下面就我院的护理体会总结如下。 1 临床资料 一般资料 选取2008 年7 月至2011 年7 月在我院住院治疗的38 例重症病毒性脑炎患儿，根据临床症状、体征、脑脊液常规、生化及细菌培养等结果，符合病毒性脑炎的诊断标准[2]。其中男22 例，女16 例；年龄10 个月至14 岁；住院天数7 天～35 天；病人均有不同程度的发热和中枢神经系统症状：其中高热32 例，头痛30 例，呕吐25 例，抽搐10 例，昏迷10 例，瘫痪3 例，脑疝2 例，视物模糊1 例。辅助检查：脑电图检查36 例，异常30 例；心电图检查30 例，异常15 例；头颅CT检查25 例，异常16 例。全部行血生化检查：23 例发生电解质紊乱，心肌酶异常27 例。全部行脑脊液检查：异常30 例，其中白细胞增高29 例，蛋白质增高10 例。 方法 对每位重症患儿均实行整体化系统护理，按护理程序的步骤：先收集护理对象的健康资料，分析资料确定护理诊断问题，制定护理计划，实施护理措施，评价护理对象朝向预期结果的进展情况。 结果 38 例患儿中治愈34 例，好转2 例，放弃1 例，死亡1 例，治愈率（34/38）。 2 护理干预措施 加强基础护理 保持病室空气新鲜，每日开窗通风3～4 次，保持床单位平整清洁、干燥。保持患儿皮肤清洁。保持口腔清洁湿润。留置胃管鼻饲患儿，每周更换胃管一次。尿失禁患儿，保持会阴清洁，留置尿管者保持尿管引流通畅，每周更换尿管一次，定时更换贮尿袋，每天用碘伏清洗尿道口2 次。保持臀部皮肤清洁干燥。 做好各种症状的护理 高热的护理 小儿重症病毒性脑炎病初即出现高热，且常呈持续性高热，。护士应密切监测体温变化、热型及伴随症状。体温超过 ℃以上者，头部可用冰帽持续降温，大血管暴露处可放置冰袋降温，四肢给予保暖，也可用低于体温2 ℃的温水擦浴，力求使体温保持在38 ℃以下。用亚低温治疗重症患儿且疗效明显[2]，应将患儿的体温控制在32～34 ℃。如高热冷敷疗效不佳者，可用药物退热。高热期要保证患儿足够营养和液体量摄入。 惊厥和频繁抽搐的护理 病毒性脑炎的患儿发生惊厥是脑缺氧、缺血及脑水肿加剧的标志。患儿惊厥发生，应去枕平卧，头偏向一侧，清理呼吸道内痰液，保持呼吸道通畅，防止窒息，置压舌板与两齿之间。必要时适当约束患儿肢体，防止坠床及其他意外伤害。患儿惊厥频繁，应遵医嘱预防性应用镇静剂，同时寻找惊厥的原因。患儿应住单间，保持环境安静，各种操作应集中进行。应观察患儿抽搐时的神志、瞳孔、抽搐发生的时间、频率、持续时间、抽搐时有无大小便失禁等。 来源：360医学网。
